At time of unannounced inspection on 9/17/24, the following physical site hazards were observed by cert rep during walkthrough of facility: Construction/renovations are being completed on the windows throughout the facility. Plywood is being used to cover the windows until the new windows are installed. Plywood covering the windows is in most classrooms and in 1 indoor play space and is accessible to children ages infant to school age. The plywood has a rough surface with splintering possibility, has duct tape around the edges that is being loosened by children tugging on it which exposes the unfinished edges of the plywood and crumbling cement underneath the tape, all of which is accessible to children. Staples that are not flush with the wall are holding duct tape in place in higher areas and these large staples are accessible to the school age children. Corners of some of the classrooms were observed to be visibly dusty and dirty from the renovations being done.

3270.151(a) · A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. 3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.
During a complaint investigation conducted on 8/12/2021 it was verified that there was a lack of appropriate supervision on 7/30/2021. Facility staff #1 acknowledged that at naptime on 7/30/2021 staff was situated in the room where she was unable to see or hear the interaction of facility child #1 and #2. At that time there was a therapist from an outside agency present in the area where the children were laying down for nap who saw and heard the conversation between facility child #1 and #2. The therapist informed facility staff #1 what was heard and seen while the staff was at the back area of the room.
