3270.64/3270.91(a) · Outside walkways shall be free from ice, snow, leaves, equipment and other hazards.Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.
During the initial inspection the rear outside walkway and path of egress was obstructed by numerous hazards and debris including used syringes, 3 electrical poles, an unstable fence that was propped by a piece of wood, two 4.5 gallons containers of sheetrock joint compound, multiple buckets and containers one filled with a brown liquid and the others filled with trash and debris, barbed wire, wooden boards and beams, open trash bags containing trash and garbage, cinder blocks, pieces of metal, excess foliage and other trash and debris.

3270.76 · Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
During the initial inspection the child care space contained two brick and mortar walls that with crumbling, open holes in some areas and gaps between the wall and the baseboard molding. The rear floor near the back door had multiple unstable areas that would sink when stepped on. The back door leading to the outdoor play space contained spray foam insulation around the door frame that was accessible to children. The window in the outdoor play space had metal on the window sill that contained sharp protruding edges. There was a square piece of wood board nailed to the wall of the outdoor play space that was chipped and contained exposed screws sticking out.
