3290.131(d)(8)/3290.131(e)(2)(i) · A health report shall include a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

At time of inspection, child #1 and #2 did not have updated immunization records with current immunization for influenza or signed letters of exemption from parents documented in files.
