On 7/11/25, at time of inspection, weeds, broken glass were observed accessible to children in the outdoor space. A section of wire fencing surrounding the playground area was damaged, with broken and bent wires protruding. The sharp, pointed edges pose a risk of injury to children. Additionally, the artificial grass carpet located on the play space was not improperly installed with edges that are rolled up due to excess length.
