Provider did not have written record of monthly testing of smoke detectors with fire drill log. Smoke detector devices located on each floor were not an enclosed non-replaceable battery smoke detectors which meet applicable UL standards and have a minimum ten-year limited warranty. At the time of renewal inspection the expiration date on the tag of the fire extinguisher located in the kitchen expired. The tag stated that it void after 1 year after service date which was 11/2020.
