During the renewal inspection, certification representative observed the facility embedded equipment in their out door play space, the equipment was not mounted over enough protective rubber covering to meet the requirements of the USCPSC, the mulch was less than 6 inches in depth around the climbing equipment and the steps to go up the slide.
