While at the facility unannounced on 9/28/23 the inspector observed that the toilet in the school aged classroom was broken. This toilet was observed to be broken on 8/31/23. On 8/31/23 the director noted that the facility was in the process of obtaining estimates for the repair. The inspector observed an odor coming from the toilet area that could be smelled upstairs as well as in the classroom. This unpleasant odor was similar to the smell of mildew.
