The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. 3. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of two months after DHS approves the documentation format.
On 5/28/25 an unannounced monitoring was conducted at the facility. It was verified by staff #1 that child #1 was left unsupervised on the toddler's playground at approximately 5:30 PM on 5/2/25. Staff person #2 had left the child behind on the toddler playground unsupervised while staff transitioned the rest of the class inside to the Movement Room during closing procedure. The child had been unsupervised on the playground for approximately 25 minutes. Staff person #2 had been unaware that child #1 had been missing from the group. Child #1 was found by the staff person #1 laying on the bench on the toddler playground at 5:56PM.
