SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

    Samantha was the lead teacher of Ella's young toddler class at the Newtown branch of the River Crossing YMCA. Her classroom assistant reported and it was observed on security camera that Samantha physically redirected Ella to stand in front of her by pulling her by her arm from behind Samantha who was sitting in a chair at the time to in front of her and then physically sat Ella down onto the floor in front of her Samantha was brought into the office after the incident was reviewed on camera and told that her actions did not meet the standard of care.

    At the time of the inspection there were multiple areas of leaking pipes observed. The bathroom next to main entrance appeared to have been flooded as well as the Bathroom in the older toddler room. The ceiling tiles in the hallway were water stained and appeared to have evidence of mold.
