Based on a self-report: On December 15, 2022 at approximately 4:00 pm, child #1 left the child care space unsupervised and alone. Child #1 walked from the second floor to the first floor and then outside the building to look for his/her parent. Child #1 mistakenly believed that their parent was enroute for end of the day pickup. When their parent did not arrive, Child #1 returned to the first floor lobby area to await their parent's arrival. Child #1 was alone and unsupervised for a period of 20-30 minutes before it was discovered that he/she had left the supervisory group of Staff #1. Also, based on a self-report: the facility had allowed children to leave the child care space on the second floor and walk to the first floor lobby area alone and unsupervised alone and unsupervised after receiving a call from the front desk stating the parents were soon arriving for pick up.

3270.113(a)(1) · Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
