In the Toddler Yellow Room, a kitchenette which was located on the left side wall had two holes in it. In the Pre-K Counts Room. A large area rug used for play was visibly dirty. In Penny's Preschool Room located downstairs two exposed screws with missing caps were protruding from the blue sensory table. In the Early Learners Room 3, a play kitchenette located in the front of the room had visible dirt on it and two holes where the oven door was removed had rough damaged plastic. In the outdoor play space a yellow truck had a sharp edge that had broken off. The pink Little Tykes Car had a broken fuel cover. A blue rocking horse was rough where the footrest was detached. The outdoor playhouse had visible dirt caked in the seats.
