    A wooden playhouse structure located in an area accessible to children in care was observed to have a broken piece of wooden trim positioned on the outside of the structure framing an open faux window. Three approximately 3" in diameter holes were observed in areas accessible to children in care on the vinyl fence enclosing the outdoor space. The edges of said holes were somewhat jagged and sharp. Contact with either the holes in the fence or the broken edge of the trim piece could cause laceration and/or puncture injuries.
