The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Child 3 and Child 8 must be dismissed from care by close of business 11/13/24 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter.
During an initial inspection on 11/6/24 Child 3, a older toddler child,(See data sheet for DOB) was missing both doses Hepatitis A vaccine. Child 8, an infant child, was missing the following vaccine doses: DTAP/DTP/TD dose 3, and Pneumococcal dose 3. At time of inspection there was no documentation on file for Child 3 or Child 8 that the child is exempt from these vaccines due to strongly held personal beliefs, religious beliefs, or medical reasons. This is not in accordance with the ACIP recommended schedule of immunizations.
