On 6/9/23, during an unannounced inspection, it was reported that on 6/5/23 at approx 12:20pm, Child #1 fell asleep at the lunch table. Staff #1 was observed smacking Child #1 on the forehead to wake up. On 5/31/23, at approx 9:30am, Child #2 took off their shoes. Staff #1 was observed using the shoe to smack Child #2 on the head before putting the shoes back on Child #2 feet. Additionally, it was reported that Staff #1 has been observed routinely using a spoon to smack children on the cheek if they play with their spoon during meals.
