    In Room #3, on the wall across from the room's entrance, there was a service disconnect panel. The electrical panel door was not locked, making two fuses accessible to children. Additionally, the main lever (throw switch) was not secured in the "ON" position. These conditions posed a potential electrocution hazard. At the back of the facility, on the deck, there was a cracked window with exposed sharp edges, which posed a cutting hazard. In the main hallway on the first floor, across from the children's restroom, there was an unlocked door leading to the basement stairs which posed a falling hazard.
