    Located in the Hallway and the Main Room there were oscillating fans that were plugged in. These fans were in reach of the children, posing a threat to any child that was in care. In the boy's bathroom was a black milk crate that was turned upside down and being used as a step stool for the children to wash their hands posing a falling hazard to the children in care. In the Preschool Room there was an extension cord running from the TV which was hung on the wall to the outlets that was close to the floor. This extension cord was loose and therefore, created a strangulation hazard to the children in care.

    In the Outdoor PlaySpace there was splintering wood on the far side of the fence along the ground creating a cutting hazard for the children in care. On the back of the storage box which held the Emergency Contact Forms and First Aid Kit in the Outdoor PlaySpace there were two metal hooks protruding out from the box creating a slicing hazard for the children in care. In the Outdoor PlaySpace along the wooden railing that separates the black top from the play equipment there were two screws protruding out creating a cutting hazard to the children in care. In the Preschool Classroom there were ten vents on the ceiling tiles that were covered in a black substance, there were four ceiling tiles that were missing pieces, and three ceiling tiles in which they were not secured in the ceiling, all of these created a crushing hazard to the children in care. In the Preschool Room there were two pipes and two metal hooks sticking out from the wall next to the right wall when entering the room. Both of the pipes and the metal hooks created a cutting hazard to the children in care. Along the far wall in the Preschool Classroom and the right-side wall the baseboard was not attached to the wall, creating pinch points for the children in care. In over ten spots on the floor in the Preschool Room, gaps in the flooring caused the movement in the flooring planks, creating a pinching hazard to the children in care. In the Hallway outside the two bathrooms, there were two pipes sticking out of the wall creating a cutting hazard to the children in care. In the Hallway there was an emergency light that was dangling from the ceiling with exposed wires and at the bottom of the steps there was an electrical box that had exposed wires that was not secured into the wall creating a crushing hazard to the children in care.

    Based on conversation with staff, the facility was notified on 5/1/23 that child #1 was treated at Children's hospital on 4/29/23. The department was initially notified of the ER treatment on 5/4/23. The child had injured their left arm while using the monkey bars in the facility's playground on 4/26/23 and at that time, there was no apparent injury to the child.

    At approximately 11 am, staff person #1 and #2 were present in the main room with 9 children, a combined group of 2 infants, 4 young toddlers and 3 older toddlers. Staff person #1 went into the adjacent infant room briefly to retrieve an item and left staff person #2 briefly out of ratio with all 8 children, as the infant room and the main room are measured as two separate spaces. A short time later, when the same staff and children were sitting down for lunch in the main room, staff person #2 went to the bathroom to grab wipes, leaving staff person #1 briefly out of ratio with all 8 children. The bathroom is off the infant room.
