The alarm to the attic door in the school age room had been turned down and would not notify staff if a child attempted to enter the attic. The screened lid to the leopard gecko tank in the school age room was ripped in many places. A Rubbermaid tote with a cracked lid creating a pinching hazard was observed on the infant/young toddler playground. A cracked flower pot was observed on the school age playground. The storage bench on the school age playground used to store toys and other play equipment was filled with mud and dirt. The door to the supply closet on the older toddler playground had fallen off. Dirty, standing water was observed in much of the play equipment on the older toddler playground. Exposed bolts were observed on the school age playground, preschool playground, and older toddler playground.
