The storage bench on the school age playground used to store toys and other play equipment was filled with mud and dirt. The door to the supply closet on the older toddler playground had fallen off. Dirty, standing water was observed in much of the play equipment on the older toddler playground. Exposed bolts were observed on the school age playground, preschool playground, and older toddler playground.
