Child #1's mother signed him out and departed at 4:35. Staff#1 and Staff #2 entered the room at 4:37, checked name/face cards and realized Child #1 was missing and checked the sign out sheet, which showed he was signed out by his parent.

Children and staff were in the circle area doing games when Staff#2 saw something out of the corner of her eye while she was sitting with those who did not participate in the game. She went and opened the door and Child#1 was standing against the wall outside.
