At time of renewal inspection on 2/11/25, during a walkthrough of the facility, damaged paint/plaster was observed in the upstairs Explorer's room along the windows/heaters and on the wall by the bathroom. A stained ceiling tile was observed on the ceiling and ceiling vents were observed to be visibly dirty.
