A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Cert Reps also observed that the still file for Staff Person #5's contain an incomplete Disclosure Statement that was not signed by a witness. Cert Reps observed that the staff files for Staff Member #4, #5, #11, #12 and #14 do not include verification of the childcare work experience amounts required to perform the duties of the position they were hired for. The Staff files for Staff #17 & #18 do not contain verification of either the educational requirements or childcare work experience amounts required to perform the duties of the position they were hired for.

3270.34(a)(1) · Tier POC: 1. A director is responsible for administering finances, personnel, maintenance, meal planning and preparation and transportation. 2. The facility owner and the facility director must attend Existing Provider Training offer by the Central Regional Office of Child Development and Early Learning. Documentation of completed training must be submitted to DHS. The operator shall provide a date of when this training will be completed.

A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.
