3270.113(a)/3270.113(a)(1) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The operator will update the existing supervision policy to include a means for staff to identify children in their groups in addition to counting - especially during times of transition. The updated policy shall be forwarded to the certification rep for approval. Upon approval, the operator will review updates with staff; each staff will sign to acknowledge understanding of the updated supervision policy. This portion of the plan shall have a date by which this will be completed.
On 1/14/26, operator self-reported non-compliance. On 1/14/26, staff # 1 left child # 1 in the bathroom for approximately 30 seconds. Staff # 1 miscounted the children assigned to them when leaving the bathroom to return to the classroom. Staff # 1 counted correctly when leaving the classroom and arriving at the bathroom; she miscounted when returning to the classroom and immediately realized she was missing a child when counting at the classroom door. Supervision policy at the facility requires staff to count children before transitioning to and from an area of the facility.
