On 1/8/26, while child #1 was having a difficult time transitioning to nap time, the child stood on other children's cots. Staff person #1 encouraged another child to stand on child #1's cot. After the child did not comply, staff person #1 stood on the child's cot. Staff person #1 then encouraged and allowed a group of children to stand on the child's cot. When child #1 noticed, he began to cry and was visibly upset. The child was upset that they made his cot dirty.

Child #1 hit their lower back on the bookshelf positioned against the changing table and then hit the back of their head on the carpeted floor. Staff person #1 failed to directly supervise child #1 on the changing table. Staff person #1 did not keep a hand on child #1 and allowed the child to stand on the elevated surface. Leadership was not notified immediately after the toddler fell from the changing table. Approximately 10 minutes later, the 2nd staff person in the room notified the director.

The facility self-reported the following incident: Staff person #1 was alone in a room at naptime with a group of preschoolers. Child #1 (5 year old) kept getting off the rest equipment. Staff person #1 forcefully placed child #1 back onto the cot. When the child tried to get up, the staff person forcefully flipped the child onto the belly. The staff person held the child down on the cot. The staff person then pulled him onto their lap and held him as the child was pulling away. The staff person eventually lifted the child from under the armpits and carried him to the door/phone area where she called for help. The incident spanned about 3 1/2 minutes.
