On 11/3/25, at approx. 11:15am, Child #1 was observed exiting the bathroom and walking down the hallway (past their classroom) unsupervised by a staff person. While walking down the hallway, Child #1 noticed certification representative at the door through the window. Child #1 stopped and waived to certification representative, ran back to the classroom and alerted Staff #1 that someone was at the door. At this time, Staff #1 left 7 preschool children unsupervised in the preschool classroom to answer the door at the end of the hallway.
