During the complaint investigation conducted on 11/4/2025 it was determined that staff person #1 accepted child #1's medication and doctor's note dated 3/12/2025 from the child's mother, to administer the medication daily during lunchtime. Staff stated that the medication was not administered due to the child's mother handing it to the staff person in their child's room on 3/13/2025 rather than the director or site operator which is facility protocol. Due to this error, child #1 did not receive this medication from 3/13/2025 to 10/21/2025.
