Continued non-compliance from 3/21/25. On 7/22/25, it was noted that weekly tracking sheets were not submitted to the Regional office since the week of 6/17/25. Facility will need to restart submission of weekly tracking sheets. In addition, management staff had not been completing daily checks, at a variety of times to monitor staff identification of primary care groups. [On 3/21/25, it was disclosed through staff interview that staff person 1 did not know that child 1 (older toddler) eloped from their primary care group. On 2/7/25 staff person 1 was using the hallway for a group activity when child 1 walked away, opened the door and entered the empty Koala classroom, which was unlocked. Child 1 then entered the storage are and started to go down the basement stairs. Basement provided potential access to small items, toxics, unsteady shelves and the hot water heater. Child 1 was discovered by staff person 3. Facility management estimates the child was unsupervised for approximately 8 seconds. Staff person 1 returned to their classroom with the rest of the children and did not realize the child was not with them until child was returned to them by staff person 3.]

On 3/28/25, it was disclosed through staff interview and review of reasons for dismissal of staff person 2, that staff person 2 appeared to be consuming alcohol while supervising children. On 3/19/25 staff person 2 was supervising 10 preschool age children by themselves when they were observed drinking something in the closet of the classroom, upon further investigation by facility staff persons 3, 4, and 7 it was discovered that the bottle contained what smelled like alcohol. Staff person 2 was not properly supervising the 10 preschool children in their care while making frequent visits to the classroom closet to have a drink an unknown substance, which is suspected to be alcohol, from their backpack.
