A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility.
Documentation showed that Staff #1 had a PA state police clearance dated 7/8/19 and a new state police clearance dated 7/12/24, which exceeds the requirement that clearances be obtained every 60 months.

Cotton balls were observed in the Monkey's classroom, where children under the 3 were enrolled. There was also a sensory tube with a top that was easily unscrewed that contained small plastic figures that posed a choking hazard in the Monkey Room. Both items were made inaccessible during the inspection. The sensory table in the gross motor space contained small cotton balls that pose a choking hazard to children who are still placing items in their mouths. Corrected on site by placing a child lock on the sensory table lid. Shredded tire pieces less than 1 inch in diameter were observed on the toddler playground.

It was observed in the young toddler classroom (room 1), a foam playmat on the floor contained tears exposing the foam causing a potential choking hazard. It was observed in the older toddler classroom (room 3) on the diaper changing table, an abundant amount of dirt and debris under the changing pad. It was also observed in the older toddler classroom (room 3), a microwave cord hanging and accessible to children causing a potential strangulation hazard. It was observed on the larger outside play space, the vinyl on the outdoor shed was cracked at the bottom left corner and accessible to children causing a potential for injury.

Peeling paint and small holes were observed on the walls in the Farmhouse Friends Room. An accumulation of dust was observed on the ceiling tiles near the vents in the Monkey Room. On the Toddler Playground, a hole in the turf surfacing was observed near the green house play equipment structure.

3270.111(c) · Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.
Child 1 was sleeping in his crib in the infant room and had a blanket around him. Staff 1 entered the room and pulled the blanket out from under/around the child to remove it, as the center policy is that blankets are not to be used in the cribs. In doing so, it startled the child awake and the child cried. Staff 1 did not then pick up the child to comfort him.

The legal entity must receive DHS approval of the training content prior to scheduling the training. The Director self-reported an incident (on PELICAN PSS) that occurred on 7/20/21. A child had reportedly fallen off of the monkey bars on the playground. The teacher with him checked him, and he appeared to not have any noticeable injuries and he continued to play. Staff filled out an accident report later, but the parent came to pick up while they were still on the playground, so they were unable to give the accident report to the parent until the next day.
