On 11/23/21 Monitoring Inspection visit there were (16) School-age children consisting of young and older school-age with one staff. The group required two staff. Staff #1(director) and Staff #2 (director) were counted in the ratio in multiple classrooms until additional staff arrived. Staff #3 a newly hired floater-staff was assigned to work in the young toddler room, when staff #1 left her alone she was not observing children and did not see when a child was repeatedly swinging a toy and hit a child in the head, or witness the child who was pushed to the floor by a rambunctious child.

On 8/12/21 at the time of this inspection the facility had 88 children in attendance. The Director was counted in the staff-ratio in the school-age classroom (25 children) located in the rear of the building. She was not able to access other areas of the building or oversee the classrooms during this time. Upon entering the toddler room, it was dark as many of the children were napping. The divided A and B room space contained a group of 6 toddlers napping in multiple areas on the floor, and there was no staff in the room. While waiting for a staff to return, I observed a teacher on the other side of the room sitting on the floor scrolling through her phone. The B-side could not be accessed from A-side without moving furniture and wakening kids. I left A-side and entered Toddler B door and observed an additional 6 young toddlers on the floor sleeping. While speaking with (B-side) staff person #1 it was revealed that she was newly hired (4-days) and that she did not know the names of the children on both sides of the room or the staff person's name who works the A-side of the room, but she did confirm that she was on her break. Observations of additional classrooms in the facility revealed young toddler and preschool classrooms were supervised by new "sub" staff (via a temporary hiring agency) hired just days and 2-weeks prior to today - 8/12/21. The newly hired staff were paired together in a classroom or were working alone, they were not paired with veteran or experienced staff to support appropriate staff-child interactions as well as guide staff on appropriate classroom practices. Of the (3) substitute files reviewed, (2) were provisionally hired. Staff #2 (after 3pm) was also working alone with a group of older toddlers. Per director (staff #3) staff meetings have not been held. Director required all staff to re-take and complete the Mandated Reporter training by 6/30/21, however, no additional trainings or meetings with old or new staff have been provided to guide them, and to ensure that safe conditions and practices are being implemented for all enrolled children going forward (per 6/18/21 POC).
