    During the inspection on 10/7/24 around 9: 57am staff person #1 allowed a child to come into the building and go to the bathroom unsupervised. Staff person #1 was standing outside of the building at the side door and could not see, direct or assess the child going to the bathroom inside the facility. Around 10:02 am staff person #2 allowed two children to go to the bathroom and the children closed the door, staff person could not see, or assess the children while going to the bathroom.
