A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1 may not work in a child care position at the facility.
At the time of inspection on 10/7/25, file of staff person #1 did not include FBI Criminal History clearance required by DHS. Staff #1 had FBI PDE clearance on file that cannot be accepted.

At the time of inspection on 10/7/25, file of staff person #1 did not include verification of education and 2 years experience required for the position of an assistant group supervisor. File of staff person #2 included only 920 out of 2500 hours of child care experience.

At the time of renewal inspection observed peeling paint on the wall and on the blue cabinet in room 3/4. Under the sink cabinet in Room 2 was damaged by the water and had peeling paint.
