The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

At the time of the investigation, it was confirmed that Staff Person #1 physically restrained Child #1 by placing both hands on the child to keep him on the sleeping mat during nap time.

At the time of inspection on 12/19/25, written diaper changing record was reviewed. According to the written record, child #1 diaper was not checked for more than 3.5 hours. Child #1 diaper was checked at 7:01 AM and then at 10:47 AM.
