A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 and #2 may not work in a child care position at the facility.
At the time of renewal inspection on 9/26/24, file of staff person #1 did not include the results of child abuse clearance from the state of New Jersey where staff person #1 resided within the last 5 years, file of staff person #2 did not include child abuse clearance from the state of Iowa, where staff person #2 resided within the last 5 years. Both staff are not provisional hire.

LACKING REQUIRED HIRING DOCUMENTS: Facility Person #3 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1, #2, #3, and #4 must be removed from child care position by close of business. Child abuse clearance must be obtained before staff person #3 may resume a child care position. Child abuse clearance and state police clearance must be obtained before staff person #1 may resume a child care position. FBI clearance and NSOR certificate must be obtained before staff persons #2 and #4 may resume a child care position.
At the time of renewal inspection, file of staff person #1 included PA state police and PA child abuse clearance for volunteer purpose; file of staff person #2 and #4 did not include FBI clearance and NSOR certificate, file of staff person #3 did not include child abuse clearance required on the date of hire. Forty five day provisional hire period for staff person #2 and #4 ended on 10/14/23. Staff persons #2, #3, and #4 were observed caring for children during on site inspection on 10/17/23.

A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
At the time of inspection, file of staff person #2 did not include an initial health assessment with TB results.
