During the course of an unannounced swimming inspection, it was observed that the first aid kits present at the off-site swimming pool were missing the following items: gloves, scissors and tape [1st grade, 2nd grade and 5th/6th grade] AND gloves [3rd grade].

3270.113(a)/3270.113(a)(1) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. [This plan must have an immediate correction date]. 2. The Facility Director must create a written document that can/should be updated and revised daily [or more frequently if needed] that account for each child in the program that specific day, to which primary care group each is assigned to and name of the staff person[s] which are assigned to that group. This document should be reviewed by and signed by the director of the program daily AND available to the Certification Representative upon request for the remainder of the swimming program. [This plan should have a specific correction date which corresponds to the last date of swimming activity].

Upon arrival, Staff Person #1 identified as the primary staff person in charge. 24-30 children were observed under the tented area and no individual primary childcare groups were assigned so that staff could identify primary care groups. One male child was observed as he walked away from the group un-noticed by staff and was prompted by CR of the child's movements. Multiple other children were also moving about unmonitored/unsupervised within the confines of the tented area.

3270.133(2)/3270.133(3) · A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Upon arrival, Staff Person #1 was asked if there were any medications for children onsite, to which it was stated that there was not. Medication for Child #1 was later found in a designated facility backpack, which had no prescription label on the package with the child's name and instructions.

Upon arrival, Staff Person #1 was asked if there were any medications for children onsite, to which it was stated that there was not. Medication for Child #1 was later found in a designated facility backpack, which was located on a picnic table within easy access of children in care. The medication was not secured in any fashion.

This is the second consecutive renewal inspection in which this regulatory citation has been made.

Upon inspection of the facility's swimming day, it was observed that staff person #1 was supervising children and counted in ratio, but did not have any documentation of a completed annual water safety instruction.
