According to documentation on file at the time of inspection, staff person #1 started working at the facility with incomplete clearance paperwork on file (see LIS code sheet for start date). Upon beginning work in the facility, the record did not contain a completed DHS FBI clearance information required under the CPSL, nor documentation the facility person had been fingerprinted for the DHS FBI clearance required for provisional hiring under the CPSL. The DHS FBI clearance was dated 13 days after start date. In addition, the NSOR was not received within 45 days and the staff person continued working. The NSOR was dated 107 days after start date.
