There is only one operator present in the facility, yet there is no system to properly supervise the children if the operator is using the restroom or preparing snacks.

Upon inspection of the facility fire system testing logs, it was found that the required tests had exceeded the maximum of 30 days between required drills on the following dates: 1.2.24/2.2.24 [31 days]; 3.1.24/4.4.24 [34 days]; 5.3.24/6.4.24 [32 days]; 6.4.24/7.5.24 [31 days]; 10.3.24/11.3.24 [31 days].

3290.151(a)/3290.192(2) · A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment on file at the facility. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Upon onsite inspection of the facility, it was observed that facility person #1 was preparing food and had entered the childcare space. No health assessment or TB test result was on file for this person.
