(9) Precautions when transporting children. (10) Pediatric first aid and pediatric cardiopulmonary resuscitation. Staff person #6 will have until 11/15/23 to complete the required training. Until such time as the required training has been completed, staff person #6 must be supervised, when interacting with children, by an AGS or GS who has completed the required training related to this citation. If there are no staff available to supervise staff person #6 staff person #6 may not work in a child-care position at the facility.

    Staff persons 1, 2, 3, and 6 did not complete professional development within 90 days of hire as listed in subsections (f)1-10. Staff person 1 - The 10 hour health and safety training was completed 132 after the date of hire, and pediatric first aid and CPR training was completed 172 days after the date of hire. Staff person 2 - Pediatric first aid and CPR training was completed 172 days after the date of hire. Staff person 3 - Pediatric first aid and CPR training was completed 215 days after the date of hire. Staff person 6 - More than 45 days have passed since the date of hire and the staff person's record did not contain documentation of completing professional development in Pediatric first aid and pediatric cardiopulmonary resuscitation.

    3270.76 · Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

    Operating fans were observed in areas accessible to the children in the gym and in the school-age space. An adult was able to get a finger through the guards at the front of the fans. The poured rubber surfacing on the playground at the rear of the building was in poor repair. The surfacing had multiple cracks, and areas where the surfacing was pulled away completely. An area on the front of a water fountain (outside of men's restroom used by school-age) was rusty and the metal was in poor repair (sharp edges). Ceiling tiles in the Brandon room showed evidence of a water leak. At least 18 tiles had water stains, and 1 tile was bulging. The protective covering along the edge of the sink in the downstairs pre-k counts bathroom was no longer covering the wood. The wood was wet and was starting to expand.
