Staff persons #1, #2, and #3 were caring for a group of 25 school-age children. 24 of the children were accounted for in care groups. The staff later identified which child was not accounted for and acknowledged his "clip" had broken the day before. Staff persons #4 and #5 were caring for a group of 21 school-age children. Staff person #4 identified being responsible for 11 of the children. Staff person #5 was unable to identify which children were in their group. Staff person #6 was responsible for a group of 8 school-age children. A child was using the restroom, and the staff person was in the hallway (unable to see the remaining children in the care group).
