    In the outdoor play space it was observed that several fence posts were cracked/chip and one post had a screw sticking out, the "music" board had a rusted slinky, sharp edges on metal wire holding chimes, vacuum tube was cracked, chicken wire with sharp edges was accessible to children, the holder for a hose had bent edges, outdoor easels had sharp/rough edges, a stump in the nature play area had rough edges. In room 104, a book was missing its cover, three lights were burnt out, and the space under the diaper changing pad was dirty. In the gym, a screw was loose in a vent and accessible to children, and the kitchen door was unlocked making it accessible to children.
