In Room 2, there was a ceiling tile that was water-stained and bowing downward (7 tiles in from the entrance). There was a second water-stained ceiling tile two further down from the bowed tile (toward back of room). In Room 1, there was a broken hands-free trashcan (corrected during inspection). In Room 2, the hands-free trashcan was not working and there was a long wire/cord with pumpkin lights (strangulation hazard) accessible to children in a bin (the long wire was removed during the inspection).It was observed, on the preschool playground, that there were 8 torn sections on the padding/mats on the ground.
