Staff persons #1, #2, and #3 were caring for a group of 11 toddlers. Staff persons #1 and #2 each identified being responsible for 5 of the children. Staff person #3 did not have a group. Only 10 of the 11 children were accounted for in the primary care groups.

On the morning of 10/13/23, two young toddlers (child 1 and child 2) were able to leave the childcare space and enter into the attached office. The toddlers closed the door to the office and were unsupervised in the space from 9:03am to 9:13am. Prior to the incident, staff person 1 was observed on video leaving the office door open before exiting the room at 9:01am. Upon exiting the room, the staff person did not transfer the supervision cards to staff person 2 (who was responsible for the toddlers). During the supervision lapse, staff person 2 was observed conversing with the other staff in the room and was not actively supervising the children in the room. Staff person 3 discovered the children were not in the room after stepping into the room to cover a bathroom break.
