It was observed in the Older Toddler (1st room on left across from office), Younger Toddler (room at end of hall perp to main hall), Older Toddler (next to staff bathroom), and the YT room (through backdoor of largest room, room on right) that there were several dead bugs in various light covers in each room. It was observed in the largest room (adjoining kitchen) that sharp scissors were accessible to children in the black cabinet. (scissors corrected during inspection). Also in this room, it was observed that there were multiple chairs stacked on each other such that they could topple over onto the children, when children were on rest mats (stacked chairs corrected during inspection). It was observed, on the outdoor play space, that there was an approximate 2'X4" gap in between the foam square pads on the ground (far end of the playground upon entering) creating a possible tripping hazard.
