Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children which includes supervision in the playground and off the premises. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a date for when this training will be completed. 3. The legal entity must develop a written supervision policy and procedure to be implemented in the facility. The written policy must address supervision in the facility, on the playground, and on excursions away from the facility. The legal entity will submit policy/procedure to the Northeast Regional Office for approval. All existing staff and new hires must be trained on the approved policy and procedure, and it must be documented in their files. The operator shall provide a date for when this correction will be completed.
On 6/9/25, staff #1, #2, #3, and #4 were on the playground at the end of the day with all the children. Child #1 guided child #2 behind block steps on the play structure out of direct view of staff. Child #1 made an inappropriate request of child #2 offering child #2 candy to not tell anyone. Staff could not recall who was responsible for supervising child #1 and child #2 on this day.
