3270.106(f) · At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.
Two children in the Older Toddler Room and three children in the Junior Preschool Room were observed to be sleeping on cots that were not spaced at least two feet on three sides. Laser measurements were used to confirm the spacing between the children's cots in both the Older Toddler and Junior Preschool Rooms. In the Older Toddler Room, Child #1's cot was measured to be 1.56 feet from a wooden wall on one side and 1.71 feet from a blue child barrier wall on the other. Child #2's cot was measured to be 1.35 feet from a wooden shelf and 1.63 feet from another child's cot. In the Junior Preschool Room, Child #3's cot had one side 1.67 feet from another child's cot and the opposite side 1.59 feet away from a different child's cot. Child #4's cot was 1.73 feet from another child's cot on one side and only 1.09 feet on the other. Child #5's cot had three measured sides which were 1.58 feet and 1.73 feet from neighboring cots, and 1.54 feet from a piece of furniture.

A ceiling tile in the Young Toddler section of the Infant/Toddler Room was observed to be chipped. Two blue cots in the Older Toddler section of the Infant/Toddler Room were observed to have cracks in the plastic. A ripped soccer ball and green colored ball were observed in the outdoor School Age Play Space. An approximately four-inch-deep hole was observed in the ground of the outdoor School Age Play Space and posed a potential tripping hazard. Trash (i.e. a plastic bag, plastic cup) and a rusted metal toy holder were observed in the outdoor Pre-K Counts Play Space.

Facility person #1 stated that children did test positive for COVID-19 at the facility on 8/3/21 and only the parents of the children who had been exposed were notified. Facility person #1 said she did not realize that all of the parents at the facility needed to be notified in order to meet the regulation.
