Based on the interviews conducted at the time of the Investigation on 11/10/2025, it was determined that Staff Person #1 and Staff Person #2, left Child #1 unsupervised in the vestibule area of the facility for at least 6 mins, when the class was returning from the outdoor play space.

The diaper changing table is positioned against a wall that prevents direct supervision in the front part of the childcare space where the classroom door and bathroom door is located. Staff #6 was not able to directly supervise Child #1 while changing diapers. During this time, Cert rep observed Child #1 pull down the exit handle of the classroom door. Child #1 was also observed unlocking the classroom bathroom door (the bathroom is shared with the neighboring classroom).
