3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Facility self-reported non-compliance: Staff 1 and 2 lined up children at the classroom door to go outside. The staff did count the children then proceeded to the outside door (which was next to their classroom door, at the end of the hallway - less than a few feet away from the classroom door). The staff opened the outside door - one staff holding the door just out of sight of the classroom, the other at the front of the line- as the children exited, stopping at the gate to the playground, to do name to face when they realized a child was not with them. The staff backed up the look in the classroom and saw the child. The child was in the classroom, unsupervised, for less than 1 minute.
