At approximately 3:05 PM in the cafeteria during supervision and ratio check it was observed that Staff Person #4 was alone with 17 school aged children, when Staff Person #4 was naming and identifying the children 3 more children entered the cafeteria from using the restroom, Staff Person #4 named the additional children and then Staff Person #2 entered the cafeteria with 2 additional children that were using the restroom. Supervision and ratio was then measured again and Staff Person #4 named and identified 12 school aged children in their group and Staff Person #2 named and identified 10 school aged children in their group, leaving all of the children accounted for.
