The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. The legal entity must arrange for all facility staff persons to complete training in proper infant sleeping position and use of age-appropriate rest equipment. Submit written verification of the completed training for all facility staff.
