On 9/24/2021 at approximately 10:50 am, staff persons #1, #2 and #3 were observed caring for a group of 14 children consisting of 7 older toddler aged children and 7 preschool aged children, in classroom 2 (located to the right as you walk into the facility.) When asked to identify the children whom they were assigned the specific responsibility to supervise, staff person #1 stated they all watched the children together and proceeded to name all the children being cared for in the classroom. When staff person #1 was asked who was assigned the specific responsibility to supervise the older toddler aged children and who was assigned the specific responsibility to supervise the preschool aged children, staff person #1 stated that he/she and staff person #2 were assigned the specific responsibility to supervise the 7 older toddler aged children and staff person #3 was assigned the specific responsibility to supervise the preschool aged children. When staff person #2 was asked to name and identify the older toddler aged children whom he/she was assigned the specific responsibility to supervise, staff person #2 was unable to name any of the older aged toddler children and stated that he/she does not normally work in that classroom. When staff person #3 was asked to identify and name the preschool aged children who he/she was assigned the specific responsibility to supervise, he/she was unable to do so and stated that it was his/her third day working at the facility.
