    3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training.

    Complaint was filed on 10/21/21 with the Department. On 10/26/21 Cert Rep did an investigation. Staff # 1 and 2 were on the playground with a group of about 7-8 preschoolers ( ages 3-5 years old). Staff # 1 and 2 did not see Child # 1-4 under the slides inappropriately touching each other private area. Complaint investigation on 11/3/21
