3270.36(b)(1)/3270.192(2)(ii) · An assistant group supervisor shall have a high school diploma or a general educational development certificate and 30 credit hours from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Staff #2 file does not contain verification of childcare experience prior to service at the facility. The experience documented from 10/3/18 does not qualify as child care experience. Staff #2, being utilized as an AGS does not qualify as an ASG. Staff #2 must be supervised by qualified staff, AGS or higher, and cannot be left alone with children.

Upon arrival at 7:08 am it was observed that there was one staff and two children present at the facility. At 7:18 am a third child arrived. The second staff person arrived at 7:22 am.

3270.113(a)/3270.113(a)(1) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

It was observed upon arrival at 7:08 Staff person #2 left the children unsupervised to open the door. This occurred again at 7:18 am. Staff person #2 did not know the names of all children present. Staff #2 and Staff #3 did not have assigned supervision groups.
