It was observed that staff person #1 answered the door, for Cert. Rep. to enter, with two children, two more children were in the child care space unsupervised. It was observed that staff person #1 walked out of the room, to the kitchen multiple times, leaving three or four children unsupervised. It was observed that child #1 walked out of the room multiple times, without supervision.

It was observed in the outside play space that there was a trampoline without a safety net, accessible to children. The trampoline also had a broken pole with sharp, rough edges posing a hazard to the children. It was also observed in the outside play space that there were several small broken pieces of toys and an abundance of trash throughout the yard and around the porch, accessible to children.
